Transportation Release

Registration Form

 January 25, 2012

Dear Senior Citizen of Ottawa County;

 If you use OCTA for your transportation needs, Ottawa County Senior Resources along with the Ottawa County Transportation Agency (OCTA) will once again be able to provide FREE trips for eligible seniors (you must be 60 years of age or older).   Included in this letter, you will find a registration form that you must complete in order to receive free transportation services.  Senior Resources must receive forms before tickets can be distributed.  REMEMBER:  Free trips are limited to OTTAWA COUNTY TRAVEL ONLY.  These FREE trips are made available through a grant from the Ohio Department of Aging and the Older Americans Act.  Once the grant funding is exhausted, FREE trips will no longer be available.

In order to meet the needs of all seniors in Ottawa County, the number of trips per client will be limited to 4 one-way trips per month.  You will need one ticket for each one-way trip.

·        First Distribution will begin February 1, 2012.

·        Second Distribution will begin July 1, 2012.

·        The maximum number of tickets given per distribution will be 24 per client.

·        If you do not need all 24 tickets in each distribution, please ask for only the total number you will need.

·        Ttickets will be distributed on a first come first serve basis.

Reservations must be made 24 hours in advance by calling OCTA @ 419-898-7433 or toll free at 1-888-898-7433.

***Rides are scheduled on a ‘First Come; First Served’ basis and the availability of vehicle and driver in your area***

Senior Resources and OCTA look forward to working with you and in the coming year.  Thank you for your support and do not hesitate to contact Senior Resources should you have any questions at 419-898-6459 or 1-877-898-6459.

Sincerely,

 Dianne Martin Mortensen, M.Ed., CTRS
